Notice of Privacy Practices
This Notice explains how medical information about you may be used and disclosed, the privacy responsibilities of Ethos Clinic, and the rights you have regarding your health information.
Effective August 25, 2026Ethos Clinic is committed to protecting the privacy and confidentiality of your health information. Federal and Pennsylvania laws may provide protections for medical, psychiatric, behavioral health, substance use disorder, and other sensitive information.
Your rights
You have rights regarding your health information. Depending on applicable law and the circumstances, these rights may include the following.
Access your records
You may ask to inspect or receive a paper or electronic copy of health information maintained about you. We will respond as required by law and may charge a permitted cost-based fee.
Request a correction
You may ask us to correct health information you believe is inaccurate or incomplete. We may deny certain requests when permitted by law and will explain a denial as required.
Request confidential communication
You may ask us to communicate with you in a particular way or at a particular location.
Request restrictions
You may ask us to limit certain uses or disclosures of your health information. We are not required to agree to every request, except when the law requires us to do so.
Receive an accounting of disclosures
You may request information about certain disclosures of your health information made during the period permitted by law.
Receive a copy of this Notice
You may request a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.
Choose someone to act for you
A person with lawful authority, such as a legal guardian or healthcare agent, may exercise privacy rights on your behalf to the extent permitted by law.
File a complaint
You may complain to Ethos Clinic or the U.S. Department of Health and Human Services Office for Civil Rights if you believe your privacy rights have been violated. Ethos Clinic will not retaliate against you for filing a complaint.
How we may use and disclose health information
Ethos Clinic may use or disclose health information as permitted or required by applicable law. Common purposes include treatment, payment, and healthcare operations.
Treatment
We may use health information in providing, coordinating, and managing your care and may share information with other healthcare professionals when permitted by applicable law.
Payment
We may use or disclose information for billing, insurance, eligibility, claims, and other payment-related activities as permitted by law.
Healthcare operations
We may use or disclose information for appropriate practice operations, including quality assessment, training, licensing, auditing, compliance, and administrative activities.
Public health and safety
We may disclose information when permitted or required for certain public health, safety, reporting, or protective purposes.
Health oversight and legal requirements
Information may be disclosed to authorized oversight agencies or as required by applicable legal process, regulation, or law.
Law enforcement and other legally authorized purposes
We may disclose health information for law-enforcement, judicial, administrative, workers' compensation, coroner, medical examiner, or other legally authorized purposes when applicable legal requirements have been met.
Your choices and written authorization
In circumstances where applicable law gives you a choice about whether health information may be shared, Ethos Clinic will follow your direction as required.
Certain uses and disclosures require your written authorization. These may include certain uses or disclosures involving psychotherapy notes, marketing, the sale of protected health information, and other purposes not otherwise permitted by law.
You may generally revoke an authorization in writing, except to the extent Ethos Clinic has already acted in reliance on the authorization or as otherwise provided by law.
Mental health and other specially protected information
Mental health records and certain other sensitive information may receive additional protections under Pennsylvania or federal law. When a law provides greater privacy protection than HIPAA, Ethos Clinic will follow the more protective requirement when applicable.
Pennsylvania law provides additional confidentiality protections for certain mental health records and limits their disclosure except when authorized by the patient or otherwise permitted or required by law.
Substance use disorder records may also be subject to additional federal confidentiality protections, including 42 CFR Part 2, when those requirements apply.
Our responsibilities
- We are required by law to maintain the privacy and security of protected health information.
- We are required to provide you with this Notice describing our legal duties and privacy practices.
- We are required to follow the terms of the Notice currently in effect.
- We will notify affected individuals as required if a breach occurs that may have compromised the privacy or security of protected information.
- We will not use or disclose health information other than as permitted by law, described in this Notice, or authorized by you.
Changes to this Notice
Ethos Clinic may change the terms of this Notice as permitted by law. Revised terms may apply to health information we already maintain as well as information we receive in the future.
The current Notice will be available on this website and upon request from Ethos Clinic.
